Keystone First Signs Value-Based Care Agreements with Nemours/Alfred I. duPont Hospital for Children and Nemours duPont Pediatrics
Keystone First, a Blue Cross Medical Assistance (Medicaid) product and the largest Medicaid managed care product in southeastern Pennsylvania, announced today that it has signed value-based care agreements with Nemours/Alfred I. duPont Hospital for Children and Nemours duPont Pediatrics in Philadelphia.
The agreements aim to improve health outcomes for Keystone First members by rewarding efficiencies and promoting accountable care. Nemours/Alfred I. duPont Hospital for Children and Nemours duPont Pediatrics will track their progress for the following performance metrics using a web-based portal that Keystone First custom-built for the two providers:
- Potentially Preventable Readmissions
- Potentially Preventable Emergency Department Visits
- Inpatient Length of Stay
- Transitions in Care
- Healthcare Effectiveness Data and Information Set (HEDIS®) Results
These agreed upon quality and efficiency measures help to improve care coordination and increase quality of care for Keystone First members.
